Enrollment documents are available online for download here. You may fax (732-747-7782) or email (enroll@americancryostem.com) your completed documents to American CryoStem. Our representatives will coordinate and schedule you with one of our physicians in your area. Your individualized collection materials will be sent directly to your physician. Payment via mail is due two weeks prior to your collection. If scheduled collection is less than two weeks, please contact us to make other payment arrangements.
You will prepare for your collection as instructed by your physician. Once your adipose tissue (fat) has been collected, it will be sent to our laboratory for processing. Both you and your physician will be notifi ed that your specimen has been received by our laboratory. Two weeks after the collection of your adipose tissue you will receive confirmation that your cells were successfully processed and cryopreserved. Your storage certificate and all documentation necessary for your records will be supplied along with instructions on how to retrieve your specimen.
